Utilizes the nursing process and principles of public health nursing and cardiovascular risk reduction to execute health promotion, early detection and prevention program activities requiring the expertise of a registered nurse.
Key elements of the role include CVD risk factor assessment, CVD health screening, patient education, and risk reduction counseling/coaching.
Assists in the collection of patient data and results communication/reporting to patient and physician.
Provides CVD related presentations and promotion of clinical preventive services to the community.
Communication/collaboration with support staff and ancillary departments.
Other duties as assigned.
Applicants Must Have:
3 years’ experience in cardiac patient care
Experience in CVD risk factor modification education
Demonstrated experience as a presenter and /or group educator
Computer skills-MS office suite
